Che cos’è la Rinoplastica e quando farla

Che cos’è l’intervento di Rinoplastica?
Partiamo subito con l’analisi anatomica del naso per comprendere meglio la funzionalità della rinoplastica.
La piramide nasale che costituisce la parte esterna del naso è formata da un’impalcatura in parte ossea e in parte cartilaginea:
- La parte ossea è costituita principalmente dalle due ossa proprie del naso, che si uniscono in alto con il processo nasale dell’osso frontale, e lateralmente con la porzione ascendente delle ossa mascellari;
- La parte cartilaginea è costituita dalla cartilagine quadrangolare, che forma il setto nasale, dalle cartilagini triangolari, o chiamate cartilagini laterali, e dalle cartilagini della punta, dette anche cartilagini delle ali nasali o alari.
Ci sono inoltre piccole formazioni cartilaginee dette sesamoidi, di numero e grandezza variabile, poste negli spazi lasciati liberi dalle cartilagini maggiori.
Le cartilagini della punta presenti una per lato, sono a forma di corolla e concorrono a formare le narici “crus laterale”, e la colonna di sostegno della piramide nasale “crus mediale”, detta columella.
Le cavità nasali, o fosse nasali, si presentano come lunghi canali separati tra loro da una parete osteo-cartilaginea detta setto nasale, le quali comunicano anteriormente con l’esterno per mezzo delle narici, e posteriormente con il rinofaringe per mezzo delle coane.
Il setto nasale è costituito:
- Posteriormente da una parte ossea formata dalla lamina perpendicolare dell’etmoide e dal vomere, da parte dell’osso mascellare e palatino;
- Anteriormente da una parte cartilaginea costituita da una lamina che, per la sua forma prende il nome di cartilagine quadrangolare del setto, e dalla crus mediale della cartilagine alare detta columella.
La parete laterale delle fosse nasali è una delle parti più importanti per la respirazione perché sono presenti i turbinati e i meati, da cui sboccano i seni paranasali, le cellule etmoidali e il canale naso-lacrimale.
Quindi, la rinoplastica o chirurgia rimodellante del naso permette di modificare la forma e le dimensione del naso per migliorare l’armonia del viso senza stravolgere la fisionomia.
Questa chirurgia del naso consente di:
- Aumentare o diminuire le dimensioni del naso;
- Cambiare la forma del dorso se incavato o con gobba o gibbo;
- Modificare la punta se troppo larga, stretta o lunga;
- Modificare l’ampiezza delle narici;
- Cambiare l’angolo tra la fronte e il naso o tra il naso e il labbro superiore;
- Correggere difetti post-traumatici o presenti dalla nascita;
- Migliorare i problemi di respirazione dovuti al setto nasale deviato, in combinazione con una settoplastica per modellare e riposizionare il setto nasale.
In base alla cronologia dell’intervento, all’etnia del paziente ed alla tecnica utilizzata la rinoplastica viene definita primaria o secondaria, etnica, chiusa o aperta.
Rinoplastica primaria, secondaria ed etnica
La rinoplastica primaria è eseguita appunto la prima volta sul paziente per correggere gli inestetismi congeniti o acquisiti.
La rinoplastica secondaria invece serve per correggere i difetti che non sono stati trattati con il primo intervento, o che addirittura sono stati provocati dalla rinoplastica precedente.
Alcuni pazienti possono aver bisogno di ulteriori interventi quando hanno subito un trauma importante o sono affetti da una grave malformazione, come la labiopalatoschisi.
Da sottolineare però che i miglioramenti estetici negli interventi successivi sono sempre meno garantiti, soprattutto se si tratta di ricostruire un naso che nel primo intervento è stato eccessivamente ridotto e presenta una scavatura.
La rinoplastica è un intervento eseguito su ogni tipologia di razza da quella caucasica, all’asiatica fino a quella africana senza alcuna distinzione.
La rinoplastica etnica però nello specifico è rivolta ai pazienti di origine asiatica o afro-americana che desiderano avere dei lineamenti più “occidentalizzati”, e l’intervento è più complesso perché i pazienti presentano delle caratteristiche di cute, sottocute, conformazione e consistenza delle strutture osteo-cartilaginee differenti rispetto ai pazienti di origine caucasica.
Rinoplastica chiusa ed aperta
In base alla tecnica utilizzata dal chirurgo la rinoplastica si divide in:
- CHIUSA, o tecnica closed, indicata per nasi che hanno pochi difetti, soprattutto sulla punta o che non presentano deviazioni della piramide nasale. Con questa tecnica si correggono bene i piccoli difetti se non ci sono grandi asimmetrie. Le incisioni vengono effettuate all’interno del naso ed il chirurgo opera con una luce frontale, e le modifiche della piramide nasale sono eseguite passando per le narici. Si tratta di una tecnica delicata che non lascia cicatrici esterne;
- APERTA, o tecnica open, utilizzata per correggere difetti importanti della punta come le asimmetrie, la punta bulbosa, il naso a pinocchio, la pinched-nose, la punta bifida, deviata o retratta. Questa tecnica è utilizzata anche per correggere esiti di labiopalatoschisi, nelle rinoplastiche secondarie o post-traumatiche. La tecnica prevede sia le incisioni all’interno del naso, sia sulla columella cioè alla base della piramide nasale e non lascia cicatrici visibili. La rinoplastica aperta favorisce il lavoro del chirurgo perché riesce a vedere i difetti delle strutture osteo-cartilaginee da correggere.
Nel caso in cui il paziente presenta delle narici molto ampie la rinoplastica può essere associata ad un’altra tecnica, che permette di ridurre le dimensioni e la forma delle narici con incisioni longitudinali o trasversali effettuate alla base del naso.
La rinoplastica può essere associata anche alla chirurgia funzionale del naso, la rinosettoplastica, cioè la chirurgia dei polipi nasali e dei turbinati.
In cosa consiste l’intervento di rinoplastica?
Per sottoporsi all’intervento di rinoplastica è preferibile aspettare la fine dello sviluppo delle strutture ossee e cartilaginee intorno all’età di 15-16 anni, per una donna l’età media indicata è 15 anni mentre per un uomo 16 anni perché la maturazione delle strutture avviene con un ritardo di 12 mesi. In ogni caso l’età di partenza necessaria per effettuare l’intervento di rinoplastica indicata è dai 18 anni in su.
Durante la prima visita il chirurgo valuta la forma e le dimensioni del naso e verifica la presenza di eventuali difetti estetici e funzionali effettuando una tomografia al naso ed ai seni paranasali, e di conseguenze illustra le possibilità di modifica mantenendo sempre un equilibrio tra forma, profilo ed estetica del naso.
La rinoplastica è consigliata nei casi di:
- Naso troppo grande in proporzione al viso;
- Naso troppo grosso o largo frontalmente;
- Naso storto o ricurvo;
- Naso asimmetrico per precedenti fratture;
- Punta del naso grossa, larga o carnosa;
- Punta del naso cadente o prominente;
- Punta del naso schiacciata;
- Naso con gobbe sia cartilaginee, sia ossee sul naso;
- Setto nasale deviato;
- Columella deviata o procedente;
- Columella retratta e labbro superiore corto.
Le correzioni vengono visualizzate tramite l’elaborazione computerizzata di foto digitali del viso del paziente, in modo da permettere la scelta più adatta alla fisionomia ed all’estetica che si vuole ottenere.
È importante anche valutare le condizioni di salute del paziente escludendo delle complicanze come la pressione alta, i problemi di coagulazione o di cicatrizzazione che possono compromettere il risultato finale della rinoplastica.
Prima dell’intervento vengono segnalate le istruzioni specifiche per prepararsi alla rinoplastica che comprendono l’alimentazione pre e post-operatoria, l’assunzione di farmaci, alcool ed il fumo, nello specifico:
- Presentarsi all’intervento a stomaco vuoto;
- Non assumere medicinali quali analgesici, aspirina e vitamina E;
- Diminuire l’uso di sigarette perché il fumo può aumentare il rischio di complicanze e causare ritardi di guarigione;
L’intervento di rinoplastica viene eseguito in anestesia generale con ricoveri in day surgery, ma in alcuni casi più complessi si necessita di un ricovero in clinica per una o due notti. Tuttavia, le rinoplastiche di modesta difficoltà possono essere eseguite in anestesia locale con sedazione.
Le incisioni effettuate non sono mai visibili perché effettuale all’interno delle cavità nasali, e questo significa anche assenza di cicatrici visibili sul naso.
L’operazione dura all’incirca un’ora ed è indolore, e termina con l’applicazione di un gesso fermato sul dorso del naso con due cerotti.
Il risultato è buono già dopo le prime due settimane post-operatorie, ma per essere quello definitivo bisogna attendere un anno dall’intervento.
La rinoplastica può essere eseguita singolarmente o in associazione ad interventi di chirurgia estetica o funzionale come la settoplastica, ma anche con blefaroplastica, lifting e profiloplastica che indica una serie di interventi (rinoplastica, mentoplastica e malaroplastica) che rimodellano in modo completo il viso attraverso la correzione di naso, mento e zigomi.
Il post-operatorio della rinoplastica
La rinoplastica è uno degli interventi di chirurgia estetica più complessi dove le complicanze come sanguinamento o infezioni sono rare, ma possono accadere se l’intervento non viene effettuato da un chirurgo specialista in cliniche specializzate.
Nelle 48 ore successive all’intervento bisogna restare in assoluto riposo con la testa sollevata, è consigliato dormire con più cuscini per favorire una buona circolazione ed evitare cibi troppo caldi o difficili da masticare.
Dal terzo giorno è possibile ricominciare con le attività quotidiane normali, evitando sforzi, saune e bagni turchi, l’esposizione solare, l’assunzione di cibi e bevande molto calde e limitando l’utilizzo degli occhiali per almeno un mese.
Nei primi giorni potrebbero apparire anche gonfiori ed ecchimosi intorno agli occhi limitabili con l’applicazione del ghiaccio, ma molto raramente il sanguinamento che necessita di tamponi.
Dopo 7 giorni dall’intervento il chirurgo rimuove il gesso dal naso che all’inizio potrebbe essere gonfio ed edematoso, ma con il passare dei giorni gonfiori ed ematomi saranno completamente riassorbiti ed assumerà sempre più la forma armonica decisa prima dell’intervento.
Il gonfiore sulla punta del naso sarà l’unico fastidio che potrà perdurare per più settimane post-intervento.
Per riprendere le attività sportivi bisogna attendere almeno un mese, affinché il processo di ossificazione del naso possa procedere nel tempo necessario senza subire traumi.
